We keep one binder for originals, one for copies, and build an appointment-specific binder so the documents needed for that person are ready to go. We carry backup documents too in case the mission asks for something we did not expect.

Scan before you mail or carry originals
We like having a clean electronic copy of important records before they leave the house. Searchable PDFs are especially helpful once an A-File or archive packet becomes hundreds of pages.
Keep reusable family records separate
A birth or marriage record can be needed again for another relative's appointment. Keeping reusable family documents organized makes it easier to rebuild a package without dismantling the original set.
Don't over-organize yourself into confusion
The goal is to find a document immediately at an appointment. A straightforward originals / copies / appointment structure is easier to maintain than dozens of tiny categories.
